Quantum Health operates Coordinated Health/Care programs for employee benefit plans throughout the nation. Coordinated Health/Care™ combines plan incentives for use of a Primary Care Physician as a “medical home,” with Patient Advocacy, Wellness, Disease Management, and Acute Care Coordination into a single, cohesive system. All customer service calls are routed through a central staff of Care Coordinators, who use an integrated IT platform to go beyond answering questions to actually coordinating patient care and guiding patients to better health. Supported by their systems, the Care Coordinators intercept patients when they are both “reachable” and “teachable” to guide them to better health within their physician’s plan of care. As a result, the Care Coordinators are actively working with 61% of the total population. With this level of engagement and interception, Coordinated Health/Care has shown a consistent ability to increase primary care and preventive utilization, and decrease ER, inpatient and outpatient utilization. Over the past 10 years, Coordinated Health/Care has held employers’ cost trend to 4.3% without cutting benefits or shifting cost, one-third to one-half the national trend for similar groups. |
